Things you need to know

Only available to eligible employees/members of Medibank corporate clients.

We’ll contact your previous insurer, with your permission, to request a Transfer Certificate. They’re required by government legislation to comply within 14 days.

You need to join Medibank within 2 months of leaving your old fund, or waiting periods apply again. Also, benefits that have been paid under your previous cover will be taken into account in determining the extras benefits payable under your Medibank cover.

^ Waiting periods apply. For ambulance attendance or transportation to a hospital where immediate medical attention is required, and your condition is such that you couldn't be transported any other way. TAS and QLD have State schemes that provide ambulance services for residents of those States.

+ Covers with Restricted or Excluded services will be treated as Included services where treatment is required for injuries sustained in an Accident that occurs after joining this cover. Treatment must be sought within 7 days of the Accident. Excludes claims covered by third parties such as Workcover and our Private Room Promise. Out of pockets may apply. Refer to your Cover Summary. For Gold hospital covers, Accidental Injury Benefit is not required because all clinical categories are Included, regardless of whether or not the treatment is required as a result of an Accident.

λ Two month waiting period applies. No annual and visit limits apply for clinically necessary dental examinations, scaling, cleaning and fluoride treatments provided by Members' Choice Advantage dentists.Maximum two bitewing x-rays per check-up, up to four per year, where clinically needed. Members’ Choice Advantage dentists not available in all areas. Different benefits and annual limits apply to other dental services included on your cover and for dental check-ups received at dentists that are not Members' Choice Advantage dentists, refer to your cover summary for details.

Φ  Benefits payable for health screening test only, excludes GP and specialist consultation fees. Services must be performed by a Medicare registered provider who is appropriate for the service and a benefit is only payable by Medibank where there are no Medicare benefits payable for those services. 2 month waiting period applies.

± Excludes Healthy Living Extras and select products that are no longer available for sale (for more information check your cover summary or check this page). Applies to prescription glasses and select contact lenses. Some glasses lens coatings and contact lenses are excluded. ∑∑  You'll need to request a private room from the Members’ Choice hospital at least 24 hours prior to your stay, provide supporting documentation to Medibank from the hospital about this request and be eligible to receive benefits for the treatment you received during your stay. Does not apply for same-day admissions or admissions for sleep studies, or where your doctor specifically requests a shared room for clinical reasons.

4 weeks free terms and conditions

 For new members on new memberships who join and start combined Corporate Medibank hospital (Bronze Plus, Silver Plus & Gold Hospital) and Corporate Extras cover (excluding Healthy Living Extras) from 9 July 2024 and who have not held Medibank health cover in previous 60 days (unless they are a dependant coming off their parent’s cover). Must quote promo code ‘CORP4PLUS’, set up and maintain direct debit when joining. Excludes resident consumer Hospital and Extras cover, Accident Cover, Ambulance Cover, Overseas Visitors Health Insurance, Overseas Students Health Cover (OSHC), Working Visa Health Insurance, ahm covers and other selected covers. Not available with any other offer, other than the applicable corporate discount. Medibank may end this offer or amend these offer terms and conditions at any time without notice.

4 weeks free terms: Must remain up-to-date with premium payments and hold eligible cover for 28 consecutive days from the policy start date to get next 4 weeks free. The total value of the 4 weeks free will then be applied to the eligible cover in full. The 4 weeks free will not be issued if you change to an ineligible cover, terminate your cover or suspend your cover during this period.

2&6 month waits waived on extras terms: When quoting promo code ‘CORP4PLUS’ when joining, 2&6 month waiting periods on extras waived. Other waiting periods apply (including 12 months on some dental services). If you’re switching from another fund and you’ve used any of your current limits (at that fund), that will count towards your annual limits with us. If you’ve reached your limits at your previous fund you may not be able to claim straight away on extras.

Live Better Points terms: Must remain up-to-date with premium payments and hold eligible cover for 28 consecutive days from the policy start date. The points will not be issued if you change to an ineligible cover, terminate your cover or suspend your cover during this period. Policyholder will require access to a smartphone and will need to download either the Live Better stand-alone app or the My Medibank App and track any Live Better activity within 28 consecutive days from the policy start. Must be 16 years or over and have a valid Australian residential address to register for Medibank Live Better. Policyholder will need to have registered a My Medibank account and linked their My Medibank account with their Live Better account within 28 days from the policy start date. Live Better points could take up to 8 weeks from the policy start date to be loaded to the policyholder’s Live Better account. 8,000 Live Better Points will be applied to Singles and Single Parent cover, and 16,000 Live Better Points will be applied to Family and Couples cover. Live Better Point component of this offer will only be applied once per eligible policy.

Live Better Rewards terms: Must be a Medibank member with eligible hospital cover, extras cover, or hospital and extras cover, be up-to-date with premium payments and have signed up to Medibank Live Better with ‘My Medibank’ or have linked their MyMedibank account with their Live Better account to redeem rewards. Additional terms and conditions may apply to the redemption of a reward depending on the type of reward chosen.  Weekly Goal Streak: Eligible Member must track and complete 10 consecutive Weekly Goal’s to complete the Weekly Goal Streak activity and earn 6,000 Live Better rewards points. Eligible Member can complete up to 5 Weekly Goal Streak activities in a year. Live Better rewards points will appear in Eligible Member’s Live Better rewards account within 30 days of completion of the Weekly Goal Streak. Live Better rewards reserves the right to cease to make the Weekly Goal Steaks available at any time. To the extent of any inconsistency between this disclaimer and the Medibank Live Better terms and conditions, the terms and conditions will take precedence.

Medibank Live Better rewards Challenges & Goals Earning Policy: The participant of a Medibank Live Better rewards Challenge or Goal may not receive Live Better rewards points or may have their already credited Live Better rewards points reversed in accordance with the Medibank Live Better rewards terms and conditions. To earn Live Better rewards points, the participant needs to properly complete 100% of the eligible Challenge or Goal according to the instructions. The number of Live Better rewards points available for Medibank Live Better rewards Challenges and Goals is subject to change without prior notice. The maximum number of Live Better rewards points that each Medibank Live Better rewards member can earn from successfully completing health and wellbeing Challenges, Goals or any Onboarding action in a calendar year is 40,000 Live Better rewards points. Special offers may apply to increase this maximum from time to time, such as for eligible Medibank members on an eligible corporate cover who complete the Weekly Goal Streaks to earn up to an additional 30,000 Live Better rewards points.
